Patient Forms

If you are new to our office and have scheduled an appointment, we will need some paperwork from you.  For each adult scheduled, we will need the Adult Info / Health History form filled out. For each child without a complicated health history, we will need a Child Info / Health History form filled out. If there are x-rays from a previous office that you would like us to retrieve, you can either call your previous office directly and request that they send them to our email (tylersmithdmd@gmail.com) or you can fill out the Consent Form so that we can request them for you. Please call our office with any questions. Thank you!
